NAPH Form Instructions
Name, Address, Phone, and Health History Form
When prompted, SAVE the form to your computer. This form is to be used at the time of an event. You must provide the requested information for each person for whom you plan to pick up medication. You will need to have multiple NAPH forms if you are picking up for more than five people. However, you should enter your health information only once and should not duplicate health information on each additional form.
